Terms for Reporting Damaged or Incomplete Orders:

understand that it is my responsibility to promptly report any damaged or incomplete orders to Active Vitality within 24 hours of receipt. Photographic documentation of the affected package or item must be submitted to sales@myactivevitality.com. Active Vitality is not responsible for items lost or stolen after the delivery is completed. Any missing or stolen items must be reported to Active Vitality within 24 hours of the recorded delivery time. For timely updates during shipping, customers are advised to use the tracking features offered by the delivery provider. Returned packages due to an incorrect or incomplete address will incur reshipment costs, which are the customer’s responsibility.

Limitations on Service and Product Availability:

Active Vitality does not make any guarantees regarding the availability or provision of services or prescribed medications. Treatment recommendations, including prescriptions, may only be issued after the customer has completed the required sign-up and has undergone a formal evaluation by a licensed healthcare provider. Medications and services are provided at the healthcare provider’s discretion throughout the program. Annual consultations are required, and lab work is requested every six months. Additional lab work may be requested by the provider at any time. Should a customer choose to use personal health insurance for lab work rather than Active Vitality’s cash-pay program, they will bear full responsibility for all associated costs.


No Return or Refund Policy:

Active Vitality strictly enforces a NO RETURN and NO REFUND policy for all medications and supplies.
