Section 1 – Patient Authorization for Medication Delivery
By accepting these Terms, you authorize Active Vitality’s staff to receive and deliver medications and refills as prescribed by your physician. Medications may be collected at the clinic or mailed to your designated address as ordered. This authorization remains active throughout your treatment unless revoked in writing. Any damaged or incomplete deliveries must be reported to Active Vitality within 24 hours, including photos of the package.
Active Vitality is not responsible for lost or stolen items once scanned as delivered. If a package is returned due to an incorrect or incomplete address, reshipment may occur at the patient’s expense.
No Guarantee of Services
No services or medications are guaranteed until completion of the registration process and an examination by a physician. Treatment decisions, including medications, are at the physician’s discretion. Patients must have an annual consultation and submit yearly lab work. Biannual lab work is encouraged but not required. Additional lab work may be requested at any time.
No Refund Policy

Informed Consent for Hormone Replacement Therapy (HRT)
The safety and efficacy of hormone therapy are subject to evolving medical perspectives. HRT is FDA-approved only for specific deficiencies; any use beyond this is considered "off-label" and carries patient-assumed liability. There is evidence that restoring hormone levels to age-appropriate norms can enhance overall health. As medical views shift, you are encouraged to discuss HRT’s current risks and benefits with your provider during annual evaluations. You acknowledge the potential risks, such as heightened chances of cancer, blood clots, or other adverse outcomes, and accept these risks in choosing HRT.

We reserve the right to refuse service at any time. Content (excluding credit card information) may be transferred unencrypted across networks and modified to conform to technical requirements. Credit card data is always encrypted. Reproduction or exploitation of any Service content is prohibited without express written permission.
